Doing the math on the health bill

In 2007 we spent $431 billion on Medicare, and we covered 41 million Americans on Medicare. In 10 years, the number of Americans age 65 or more will increase by 36% to 54 million, and the health bill whcih is about to be signed into law says that we will decrease Medicare spending by $500 billion over 10 year.

Can someone please help me to understand the math?
